Here's a good example.

Meet Lisa! During a meeting with her dermatologist about a pesky rash, she said...

“While I’m here, can you remove a splinter in my finger? 

“Sure!” he said and popped out the splinter in no time. 

The bill?  $1100,  just for the splinter
Why? The doctor made a tiny cut so it was billed as outpatient surgery. Another billing code would have covered splinter removal at $58.00. This practice is called upcoding and it happens way too often.
